Just wanted to hear what else Bob had to say about the ActIndex issue because if I understood him correctly when I first asked him, he has already switched off of the deprecated SetWeaponHoldType method, yet the error still persists?

Oh. Well.
Apparently what happened is that the update Kilburn released regarding holdtypes was actually supposed to fix holdtypes so that they would be properly predicted (if I remember correctly). Instead of using weapon:SetWeaponHoldType, the new way is to use SetHoldType, which calls setweaponholdtype internally and in sync on both server and every client or something like that. This is why before you'd have weapon bases that would change holdtypes for the gun depending on what the user was doing (aiming down sights, reloading if the coder wanted to use a different reload animation), but only the LocalPlayer could see them.
